Pediatric Healthy Weight Case Management Program

The Pediatric Healthy Weight Case Management Program was developed to help Pitt County children and their parents find the care and resources to reach and stay at a healthy weight.

The program uses a team approach to provide children (up to age 18) and their families with skills and information to maintain a healthy weight. It encourages patients, families and their doctors to work as a team along with a clinical social worker, a dietitian and registered nurses who help manage each child's care.

With help from other specialists, the team develops an individual, written treatment plan to help each child

  • Maintain or decrease body mass index (BMI)
  • Control or improve conditions such as diabetes and high blood pressure, which occur in overweight children
  • Increase exercise and improve diet
