The
Pediatric Healthy Weight Case Management Program was developed to
help Pitt County children and their parents find the care and
resources to reach and stay at a healthy weight.
The
program uses a team approach to provide children (up to age 18) and
their families with skills and information to maintain a healthy
weight. It encourages patients, families and their doctors to work as
a team along with a clinical social worker, a dietitian and
registered nurses who help manage each child's care.
With
help from other specialists, the team develops an individual, written
treatment plan to help each child
- Maintain or decrease body mass index (BMI)
- Control or improve conditions such as diabetes and high blood pressure,
which occur in overweight children
