Community Care Plan of Eastern Carolina
About Us
Community Care Plan of Eastern Carolina (CCPEC) is one of the 15
regional networks within the state's Community Care Plan of North
Carolina. The networks consist of primary care physicians and community
organizations such as public health, social services and hospitals,
which have voluntarily come together in a commitment to improve access
to quality healthcare and to improve health outcomes for the Medicaid
population.
How can we help?
- By coordinating care, providing education about diseases and helping the client set realistic goals for better health.
- By ensuring well visits and follow-up care with client's healthcare provider to maintain medical standards of care.
- By introducing clients to resources in their area as well as teaching them how to access and use these resources.
- By helping clients manage and set up Medicaid status and transportation.
- By receiving referrals from healthcare providers, other healthcare organizations, schools, Medicaid database and self referrals.
